teacher who is eligible for the Outstanding Teacher Program.

C. Statement of your goal relative to
standards implementation
1. Name and describe your goal
2. Focus on 1 or 2 standards (2 is
maximum) and the checkpoints appropriate to your teaching
assignment (note: for this purpose in Science or History, standard
1A or 1B, for example, would each count as 1 standard. It is not
necessary to do both to meet the 1 standard minimum requirement.)
D. Brief thesis statement explaining
why you believe your work this year in implementing Standards Based
Education is at the outstanding level of performance.
III. Criteria and
Artifacts*
Provide artifacts to demonstrate
performance at the V or IV variance for each identified component in
the Configuration Map. At least one component must be demonstrated
at the V variance. (See: District Document, "Standards-Based
Education: Configuration Map, February 1999" for additional
information)
At least one artifact must be
provided for each component (A,B,C & D as listed below) plus 2
additional artifacts of your choice for a total of 6 artifacts.
